Our services are available to individuals and families who meet certain eligibility requirements. Our payment options are Medicare, Medicaid, Insurance and Private Pay.
Medicare: According to Medicare.gov, to qualify for medicare, you must be 65 years or older and a citizen or permanent resident of the United States. However, if you are not up to 65, you might also qualify for coverage if you have a disability or with End-Stage Renal disease (permanent kidney failure requiring dialysis or transplant).
Medicaid: To be eligible for Medicaid, federal and state requires an individual to satisfy certain requirements. These includes:
- Be age 65 or older and/or adults with disabilities
- Have a permanent disability
- Be a U.S. citizenship or meet certain immigration rules
- Be a resident of the state where you apply
To qualify for medicaid, a medical specialist in your state of residence must evaluate your needs and make a decision on whether a long-term care service is needed.
Insurance: We also accept insurance based on the insurance coverage. Coverage may vary and we encourage you to contact your coverage provider, or our office, and we will assist you in determining your benefit coverage. We accept the following insurance:
